Event Registration

Event registration fee is $50/bowler (early bird registration fee is $40/bowler if received on or before 1/26/24)
$15 for expenses
$20 for donation to Susan G. Komen ($10 for early bird)
$15 for prizes

Since this event is open to adult and youth, we can only accept individual entries. If you plan on bowling with a team, you can include them in the form when you add this product to the cart. However, each bowler must pay for his/her entry separately. Including a person’s name in the team form does not reserve that person a spot at this event.

If you are submitting 16 or more paid entries for the same squad, please use the printable PDF Entry Form and you will receive one (1) free entry into the tournament.

Entries submitted by mail must be paid via check ($25 fee for NSF checks) or Zelle.
Entries submitted electronically must be paid via Zelle.
Zelle payments should be sent to office@chicagobowling.com.
